Comments:

Pin 2 should not be connected at both ends, the whole point of MIDI using opto-isolators is to avoid earth loops caused by having individual synths earthed at more than one point -- through the audio cable and again through the MIDI cable.

For _cables_, pin 2 should be connected to the shiedling of the wire on both ends - the jacks on the instrument side deal with the connect / dont connect issue. The shell of the plugs, on the other hand, should _not_ be connected to the shielding.
